The major focus of FOR YOUR HEALTH (4YH) is a marketplace connecting healthcare professionals, facilities and patients seeking traditional and specialised medical services, including home care. The portal allows users to easily find the desired services by filtering for geographical proximity and type of assistance required. Partners are selected and validated via blockchain technology. 4YH offers services with a high social impact, such as animal-assisted therapies, hippotherapy, music therapy, transport for the disabled, home care for the differently-abled and psychiatrically ill, companion care services for the elderly and care-dependent people, and healthcare professionals such as nurses, physiotherapists, osteopaths and home carers. The business model includes the acquisition of private households and profiling data, as well as the development of B2B channels for consulting, big data and digital marketing. The portal is currently being finalised.<\/p>\n

\nIntegrated home care services – Integrated home health and social services are essential for chronic and dependent patients.<\/p>\n

Why is it important?<\/strong><\/h3>\n

The system offered to families is characterised by territorial penetration and the profile of dedicated experts: families can count on the experience of a team of professionals and the opportunity to support specific needs by shortening distances. We have already aggregated a network of vendors with a widespread presence in the territory from which we intend to start, partners with unexpressed potential that can therefore be activated in a short time. The market analysis carried out in the area has allowed us to map the specific needs of customers and to receive adhesions of about a hundred private facilities and professionals in Piemonte, who have signed a letter of intent. The sectors with which we have already signed agreements are the following: Pet therapy, hippotherapy, accompanying services, legal and bureaucratic services, care services for the mentally ill and children with disabilities.
\nOur challenge is always the customisation of services: we offer different packages according to needs (economic availability, level of non-self-sufficiency, family condition). Home care does not take into account the overall needs of non-self-sufficient persons who have a lasting need for help, also and above all in performing the daily activities. Moreover, there are generally no 24-hour support, counselling and information systems for those home care patients who may have urgencies that, if solved, could avoid inappropriate hospitalisation.<\/p>\n
